Make an Appointment
Student Health Services is not a walk-in clinic. You require an appointment to see a physician, however, we can often accommodate same-day appointments. Our appointment-based system limits wait times, allowing us to better serve you.
We thank you for your patience because we are currently experiencing a high demand for service.
Helpful Information About Appointments
1) Our appointments are booked on a first come first served basis.
2) All morning appointments are pre-booked in advance.
3) Afternoon appointments are saved for same day appointments.
4) If you require a same day appointment, please call as close to 9:00 am as possible. Please note, you may need to wait on hold as you are put in a queue on the phone.
5) If all appointments are booked for the day and you have a significant concern you can:
a) come in-person and wait to speak with information nurse from 11:00-3:30 Monday to Friday.
b) call Health Services at 519-661-3030 and press option #3 to speak to the information nurse.

Making an Appointment
To make an appointment, simply contact Student Health Services, either by stopping by the clinic if you're on campus or by calling us at 519-661-3030 (or extension 83030 from a campus phone). If you arequire a same day appointment please contact us as before 11 AM. Alternatively, you can also request a specific date and time that is convenient for you in the near future. You can requrest the gender of your doctor or ask for a specific doctor if you have used our services before.
When making an appointment, you will be asked for:
- Your student number
- Your current address, telephone number and e-mail address
- Your health card
- A reason for your visit

Arriving for Your Appointment
Unless otherwise instructed, please arrive for your appointment ten minutes early. You must check in with reception or using our Self Check-in computers. You must also bring your health card to each appointment.

Missed or Cancelled Appointment
SHS requires adequate notice for cancellations. Missed appointments or appointments cancelled without adequate notice will result in a no-show/late cancellation fee being charged. Students who arrive more than five minutes late for an appointment may also be charged for a missed appointment. Click here for the cancellation policy.

Accomodation for Medical Illness
The University of Western Ontario requires documentation from the medical community for students who require academic accommodation due to medical illness. For complete details, please see the Policy on Accommodation for Medical Illness. Students are required to have their physician or health care provider fill out the Student Medical Certificate. At Student Health Services the fee for having this form filled out is $20. Students seeking academic accommodation should request a UWO Student Medical Certificate to be filled out at the initial visit with the physician. Medical certificates will only be considered for students seriously affected by illness, who can not reasonably be expected to meet his/her academic responsibilities. Medical certificates may NOT be issued for minor illness like colds, non-acute vomiting or diarrhea, menstrual cramps, insomnia or for past illnesses not seen at student health services. If the requested accommodation is related to a death in the family, please see your academic counsellor directly.

Obtaining a Student Medical Certificate at Student Health Services
1) If medical concern arises that significantly impacts academic performance, at time of illness make an appointment with a physician at Student Health Services. 2) During this appointment, request a Student Medical Certificate, from the Physician. 3) Go to Accounts Office (room 19) at Student Helath Services for payment and pick up of form. 4) The form contains a section for student signature. This needs to be filled out ONLY if further documentation is required.
